  Super Selective LASER Lipolysis & Suction (LASER Liposuction)  
  Liposuction is a procedure that can help sculpt the body by removing unwanted fat from specific areas, including the abdomen, hips, buttocks, thighs, knees, upper arms, chin, cheeks and neck.

During the past decade, liposuction, which is also known as "lipoplasty" or "suction lipectomy," has benefited from several new refinements. Today, the older techniques like ultrasound-assisted lipoplasty (UAL) are being replaced by newer techniques like power assisted lipoplasty (PAL). These latest techniques are helping plastic surgeons to provide  patients with more precise results and quicker recovery times. Although no type of liposuction is a substitute for dieting and exercise, liposuction can remove stubborn areas of fat that don't respond to traditional weight-loss methods.

We can remove unwanted fat from any body area, the commonest being abdomen, love handles, thighs, back and arms. The other common areas are male of female breast, double chin, neck and face.

Dr. Patankar uses the latest technique called LASER LIPOSUCTION. This technique has replaced the earlier technique of Ultrasound Assisted Liposuction or UAL. As of date there are very few centres in India offering this latest technique.

The time required to perform LASER LIPOSUCTION may range about half an hour to a couple of hours depending on the magnitude of surgery.

Liposuction is a procedure in which localized deposits of fat are removed to re-contour one or more areas of the body. Through a 2 mm puncture wound, a narrow tube or cannula is inserted under the skin in a fat layer. A fibre- optic system delivers the LASER beam and melts the fat. The LASER beam also clots the fine blood vessels in the fat layer and hence there is minimal bleeding. A high powered suction is the then used to vacuum the fat layer that lies deep beneath the skin. If many sites are being treated, your surgeon will then move on to the next area, working to keep the incisions as inconspicuous as possible.


Ultrasound-Assisted Lipoplasty (UAL) : This is one of the older techniques rarely used today. This technique requires the use of a special cannula that produces ultrasonic energy. As it passes through the areas of fat, the energy explodes the walls of the fat cells, liquefying the fat. The fat is then removed with the traditional liposuction technique. This technique suffered due to certain risks like skin necrosis and scarring, uneven contouring, fat embolism etc.
Power Assisted Lipoplasty (PAL) : UAL has now been replaced by the latest technique of Power Assisted Lipoplasty (PAL). This technique employs a very high frequency oscillating cannula which breaks the fat cells. This has overcome the risks of UAL and made the procedure much safer and results more refined. It is still used today to do large volume mega liposuction.
LASER Liposuction: This is the latest technique in the world. As mentioned earlier, this technique uses LASER to melt the fat. It enables the surgeon to break a touch localized fat as in male breast or gluteal and upper back region.
The real big advantages of this technique(LASER Liposuction) are, minimal blood loss and hence consequently ability to do very large volume mega- liposuctions, safety possibility of surgery under local anesthesia, very fine and controlled, minimal post operative pain and fast recovery.

To be a good candidate for liposuction, you must have realistic expectations about what the procedure can do for you. It's important to understand that liposuction can enhance your appearance and self confidence, but it won't necessarily change your looks from an excessively obese to a very thin individual.

The best candidates for liposuction are normal-weight people with firm, elastic skin who have pockets of excess fat in certain areas. You should be physically healthy, psychologically stable and realistic in your expectations. Your age is not a major consideration; however, older patients may have diminished skin elasticity and may not achieve the same results as a younger patient with tighter skin.

Extra care needs to be taken in individuals with medical problems such as diabetes, significant heart or lung disease, poor blood circulation, or those who have recently had surgery near the area to be contoured.


The initial consultation with your surgeon is very important. The surgeon will need your complete medical history, so check your own records ahead of time and be ready to provide this information. Be sure to inform your surgeon if you have any allergies, if you're taking any vitamins, medications (prescription or over-the-counter), or other drugs; and if you smoke.

During your initial consultation, Dr. Patankar will personally evaluate your health, determine where your fat deposits lie and assess the condition of your skin. He will explain the body-contouring methods that may be most appropriate for you. For example, if you believe you want liposuction in the abdominal area, you may learn that an abdominoplasty or "tummy tuck" may more effectively meet your goals; or that a combination of liposuction and tummy tuck would be the best choice for you. He would further explain the techniques and anesthesia he will use and the risks and costs involved.

You and your surgeon should carefully discuss your goals and expectations for this surgery. You'll need to discuss what all areas of your body need the contouring and whether any adjuvant procedures like fat transfer or breast reshaping would enhance your body contour.

Don't hesitate to ask your doctor any questions you may have, especially those regarding your expectations and concerns about the results.


Dr. Patankar will give you specific instructions on how to prepare for surgery, including guidelines on eating and drinking, smoking, and taking or avoiding vitamins, iron tablets and certain medications. If you develop a cold or an infection of any kind, especially a skin infection, your surgery may have to be postponed.

Also, while you are making preparations, be sure to arrange for someone to drive you home after the procedure and, if needed, to help you at home for a day or two.


Liposuction may be performed in a surgeon's office-based facility, in an outpatient surgery center, or in a hospital. Smaller-volume liposuction is usually done on an outpatient basis for reasons of cost and convenience. However, if a large amount of fat is removed, or if the liposuction is being performed in conjunction with other procedures, a stay in a hospital may be required.


Various types of anesthesia can be used for liposuction procedures. Together, you and your surgeon will select the type of anesthesia that provides the most safe and effective level of comfort for your surgery.

If only a small amount of fat and a limited number of body sites are involved, liposuction can be performed under local anesthesia, which numbs only the affected areas. However, if you prefer, the local is usually used along with intravenous sedation to keep you more relaxed during the procedure. Regional anesthesia can be a good choice for more extensive procedures. One type of regional anesthesia is the epidural block, the same type of anesthesia commonly used in childbirth.

However, some patients prefer general anesthesia, particularly if a large volume of fat is being removed. If this is the case, anesthesiologist will be called in to make sure you are completely asleep during the procedure.


After surgery, you will likely experience some fluid drainage from the incisions. Occasionally, a small drainage tube may be inserted beneath the skin for a couple of days to prevent fluid build-up. To control swelling and to help your skin better fit its new contours, you may be fitted with a snug elastic garment to wear over the treated area for a few weeks. Your doctor may also prescribe antibiotics to prevent infection.

Don't expect to look or feel great right after surgery. Even though the newer techniques are believed to reduce some post-operative discomforts, you may still experience some pain, burning, swelling, bleeding and temporary numbness. Pain can be controlled with medications prescribed by your surgeon, though you may still feel stiff and sore for a few days.


Liposuction is normally safe, as long as patients are carefully selected, the operating facility is properly equipped and the physician is adequately trained.

As a minimum, your surgeon should have basic (core) accredited surgical training with special training in body contouring. Also, even though many body-contouring procedures are performed outside the hospital setting, surgery in a hospital is always a safer option.

Your doctor must have advanced surgical skills to perform procedures that involve the removal of a large amount of fat (more than 5 liters)

Risk of anesthesia:
If only a small amount of fat and the limited number of body sites are involved, liposuction can be performed under local anesthesia, which numbs only the affected areas. However, if you prefer, the local anesthesia can be used along with intravenous sedation to keep you more relaxed during the procedure. Regional anesthesia can be a good choice for more extensive procedures. One type of regional anesthesia is the epidural block, the same type of anesthesia commonly used in child birth. In this technique a very small injection is given in the back, not in spinal canal (spinal anesthesia) but outside the spinal canal. Thus "epidural anesthesia" is different from "spinal anesthesia and does not cause any backache etc.

Some patients prefer general anesthesia, usually out of anxiety. However the rest in general anesthesia is always more than the regional or local anesthesia. It would be most advisable to leave the choice of anesthesia to your anesthetist.

The latest technology of LASER liposuction causes very minimal blood loss. However do remember and give history of any of the following things to your doctor,

1. Whether you have bleeding disorder?
2. Whether you have received blood transfusion for whatever reason in the past?
3. Do you have a rare blood group like RH negative?
4. Whether you are on any medications esp. aspirin, anticoagulants, vitamin-e, contraceptive pills etc?

Dr. Patankar has not given blood transfusion even to the single patient after liposuction in the last 15 years. Normally we don't even need to send blood to 'Blood Bank'.

Hematoma or Seroma formation:
Hematoma is collection of blood under the skin and Seroma is collection of fluid and liquefied fat. You would be given compression garments immediately after the surgery to avoid these complications. However some temporary minimal bruising of the skin is normal and would disappear within a week or so.

Infection is a theoretical possibility with any surgery and so also with this surgery. However, everything is used for surgery is disposable and you would be prescribed antibiotics for a weeks after the surgery.

Skin Ulcerations or scarring:
In older version of liposuction viz 'Ultrasonic liposuction, the heat from the ultrasound devise, used to liquefy the fat cells, could cause injury to the skin or deeper tissues leading to bad scarring. In contrast PAL is much safer technique, the safest being LASER liposuction.

Perforation of intestine / damage to internal organs:
Your surgeon needs to be experienced enough to avoid these complications, he would evaluate the tone of the abdominal muscles and presence of abdominal hernia if any. Abdominal muscles protect bowls hence any patient with poor muscle tone needs extra care. The surgeon needs to note and repair abdominal hernia.

Fat embolism:
This is a life threatening complication was associated with older types of liposuction, like Ultrasonic liposuction.  In UAL a blast of ultrasonic waves would rupture fat cells. However there was possibility of these fats entering blood vessels under force of a blast and get carried away like clot. This fat could lodge in vital organ and cause problems. However not a single case of fat embolism has been reported anywhere in the world following LASER lipolysis. Thus this latest technology is extremely safe.

Every patient gets some swelling over the operated area after 2 to 3 days. This goes down gradually as the skin shrinks over two months. So you would notice an immediate reduction in your bulges after liposuction and the result would continue to improve over a couple of month.

Numbness of skin:
The operated area feels slightly numb and the sensations return to normal as the swelling reduces. Most patients are happy about, it because this makes the post - operative period painless.

Skin irregularities or waviness of the skin:
This is a completely avoidable complication. It is important for the patient to wear pressure garments after the surgery for the skin to shrink evenly and smoothly. The skin surface may become irregular, asymmetric or even "baggy" especially in the older patients, particularly so if they don't use pressure garments.


However, it's important to keep in mind that risk increases if a greater number of areas to be treated at the same time, or if the operative sites are very large in size. Removal of large amount of fat and fluid may require longer operating times and would pose higher risk than that for smaller operations.

Hence it would be advisable to let the experience of your surgeon decide what is safe for you. He would certainly remove the maximum possible fat in one go, however some other remaining body areas can always be treated at a later date. 

There are no long term complications after liposuction and hence it is perfectly safe even in unmarried girls and boys. There are no problems concerning functioning of internal organs or future pregnancies.


Healing is a gradual process. Your surgeon will probably tell you to start walking around as soon as possible to reduce swelling and to help prevent blood clots from forming in your legs. You would most likely go home by evening on the day of the surgery and be back at work within a couple of days. There are usually no stitches and even if there are a couple of them, they would dissolve on their own within the first week.

Vigorous and strenuous activity should be avoided for about a month as your body continues to heal. Although most of the bruising and swelling usually disappears within three weeks, some swelling may remain for two months or more. You would be advised to wear a compression garment for a variable period ranging from 3 weeks up to 3 months.

Your surgeon will schedule a follow-up visit after about a week to monitor your progress. A late follow-up visit after a couple of months is advised to see the final result.

Adipocytes (fat cells) are physiologically of two types. Slow Metabolisers (SM) and FAST Metabolisers (FM). SM cells burn fat slowly and tend to store fat while FM cells tend to burn fat rather than storing it. The distribution of these cells in the body is decided genetically and hence a pattern of obesity tends to run in families.

Suppose you have SM cells storing fat on the abdomen and mainly FM cells on the rest of the body areas. In that case you would have a tendency to put on over the tummy and the rest of the body would be in relatively normal proportions. If we were to remove fat cells from your tummy by liposuction, the type of fat cells from the rest of the body would remain unchanged. Hence you would have no fear of putting on anywhere else even in the long run. However if you have a generalized obesity, it would be advisable for you to adopt a better and healthier life style.


You will see a noticeable difference in the shape of your body quite soon after surgery. However, improvement will become even more apparent after about four to six weeks, when most of the swelling has subsided. After about three months, any persistent mild swelling usually disappears and the final contour will be visible.

If your expectations are realistic, you will surely be very pleased with the results of your surgery. You may find that you are more comfortable in a wide variety of clothes including the western outfits and more at ease with your body. You may have to alter your wardrobe to adjust to your disappearing flab and shrinking waistline. You would once again be able to wear all those clothes in which you could not fit for all these years. You would start getting complements for the 'FIT' body. With improved looks, you would be more confident at work and at home. You would find yourself enjoying the life to the fullest once again.
